Hospice at Home and in Long-term Care

Hospice provides compassion and hope

The end of a person’s life is a significant time when they are able to reflect on their life and accomplishments, work through their regrets and challenges, and spend their remaining days with friends and family in comfort and peace. For families whose loved one is experiencing a serious illness or life-limiting condition, this precious time is one of life’s most meaningful milestones.

Kansas City Hospice & Palliative Care provides expert, compassionate care wherever you call home. Our innovative programs put us at the forefront of how individuals should be cared for at the end of life and how families should be supported. We serve families in homes and long-term care facilities across 12 counties in Missouri and Kansas.

Our unmatched expertise ensures pain and other symptoms are managed, so patients can enjoy a high-quality of life at any stage of illness. Specialized multidisciplinary care teams ensure each person’s physical, emotional, mental, social, and spiritual needs are met, so they can enjoy life comfortably with their loved ones.

Ensuring comfort and quality of life, wherever you call home

When the focus shifts to comfort instead of treatment, hospice at home helps the whole family focus on what matters most. Hospice prioritizes quality of life and helps families enjoy their time together in comfort and peace.

  • Regular visits provide specialized care and compassionate support
  • Alleviate pain and other symptoms that cause discomfort
  • Nurses on call 24/7 for questions or emergent needs
  • Individualized care helps each person focus on what is most important to them
  • Whole-person approach addresses emotional and spiritual needs and provides support to family, including through the grief process
  • Help patients stay independent longer, spend less time in the hospital, and greatly reduce ER visits

This same level of care is available for those whose home is in a long-term care facility. We’ll collaborate with the staff at your facility to help meet your needs and goals, and ensure you are able to stay independent longer.

  • Regular visits compliment the care your facility provides, with specialized expertise in comfort care and end-of-life symptom management
  • Dedicated long-term care teams have established relationships with each facility’s staff
  • Assistance communicating with family members, whether local or out of town
  • Your KCH team will work closely with facility staff to adjust care as needs change

What to Expect - Hospice at Home and in Long-term Care

Kansas City Hospice & Palliative Care provides each patient with a RN case manager and a multidisciplinary team of hospice professionals. You will have the ability to choose which services you or your loved one receives. Our experienced staff caregivers will visit as often as needed and desired. Most families choose to receive care from several team members, including social workers, certified nursing assistants, chaplains, expressive therapists, volunteers, grief support specialists and more.

Your own doctors will stay involved in your care and your personal physician will direct the medical care you are given. Specialized KCH physicians, who are experts in pain and symptom management, are available for consultation.

The Medicare Hospice Benefit covers 100% of your costs for care and most insurance providers offer a similar benefit. We pride ourselves on providing the highest-quality hospice care  in the region, and on ensuring this same level of care is accessible to all who need it, regardless of financial resources or complexity of need.
