From palliative home care, to hospice, to hospice house — Heart Matters follows your cardiac patient through a full continuum of care.
When patients have cardiac disease, they may need extra support at home. Heart Matters is an extra layer of care on top of the care you give. We go to them wherever they live to help improve quality of life and help the whole family cope with cardiac disease.
Studies have shown that palliative and hospice care reduce time in the hospital, help avoid emergency room visits and even extend life.
Services are available for many situations, from one-time consultations to regular, ongoing visits, even for patients not ready for hospice.
An RN certified in heart disease coordinates care, and your patient can choose to receive care from other team members, such as home health aides, social workers, chaplains, dietitians, art/music therapists and more.
We’ll work with your patient right in their home to help them comply with your plan of care:
- Scheduled visits with palliative or hospice team
- In home cardiac care kit for quick access to medication
- Monitoring of symptoms, fluid balance and oxygen levels
- Education tailored to patient and family
- Home lab work, physical therapy and personal care, as needed
- Emotional and spiritual support with a social worker and chaplain, if desired
- Round-the-clock access to nurses and emergency home visits
Payment and eligibility
Medicare, Medicaid and most private insurance plans pay for palliative or hospice care from Heart Matters. We will help your patients find out what their insurance covers or if they qualify for subsidized services.
