Programs for Patients at Every Stage of Serious Illness

Most patients with serious illness don’t consider palliative or hospice care until it’s too late in the disease process for both patient and family to receive the full benefit of the programs. We want to partner with you early in the process to help address both medical needs and quality of life. Our programs and services are designed to help you provide the best possible care, starting while you are still providing curative treatment and helping you support your patient and their family with an interdisciplinary team that helps implement your plan of care.

Palliative care during serious illness – the area’s first palliative care team to provide patients with relief from the symptoms, pain and stress of serious illness, whatever the diagnosis. Patients feel better, families cope better and patients may live longer. Patients may be receiving active care, or not be ready or appropriate for hospice.

Transition to hospice – our hospice care teams will help you provide your patients with the care and support they need as their disease progresses and needs change. Our coordinated approach helps reduce patient anxiety and hospital admission, increasing both the quality and quantity of life, while also giving support to the family.
