Advanced Illness Support

Advanced Illness Support is a program to provide expert advice and support in the comfort of your own home. Our experienced Nurse Practitioners are uniquely qualified to help manage your symptoms and keep you comfortable.

When you’re dealing with a serious illness, you may need extra support at home. People with cancer, heart disease, respiratory disease, kidney failure, dementia, neurological disease and many other illnesses may all benefit from palliative care.

Services are available for many situations, from one-time consultations to regular, ongoing visits.

We can help as early as when you received a diagnosis or at any stage of your illness. We’ll help you to:

  • Be more comfortable – We’ll help control pain, shortness of breath, nausea, trouble sleeping and other symptoms that come with your illness.
  • Understand your illness – Information gets thrown at you very quickly. We’ll help you understand what it all means and be there to answer all your questions.
  • Feel less overwhelmed – We’ll help you understand your body’s reactions, medications and options so you can make informed decisions. We’ll teach your family how to help you.
  • Worry less – You can discuss choices and plan ahead for possible changes, like going back to a normal routine, going to the hospital if needed and deciding what to do if your condition changes.
  • Coordinate care – We’ll work with your doctors and other health care providers to make sure everyone is kept informed and understands your goals. You keep your doctors and have access to our palliative specialists (nurse practitioners and licensed clinical social workers), as needed.

Our nurse practitioners are highly-trained experts in palliative care. Your NP will:

  • Provide  intermittent home visits based on needs
  • Help you and your family understand your illness and options for care
  • Coordinate care with your doctors and other providers
  • Recommend how to best cope with pain and other symptoms
  • Teach you skills that will help keep you comfortable at home
  • Plan for future needs
  • Identify when you qualify for more help at home
